At North Shore Eye Care, here in our Smithtown, Long island, we perform the most advanced cataract surgery available. Cataract surgery has come a long way in the last thirty  years. Through the use of conscious sedation, an incision under three millimeters is made in the cornea and the cataract is removed using high speed ultrasound (phacoemulsification). The entire procedure usually lasts about ten minutes. In our practice, needle injections around the eye are rare used and hence  we do not routinely use sutures or patches. This gives our patients who are on anticoagulation (Coumadin, Plavix, Aspirin)the ability to continue there medicine prior to and after surgery. Our patients usually have quick recovery and are able to see right away.

We are happy to offer the latest in intraocular lenses in cataract surgery. Dr. Lawrence Zweibel, Dr. Jeffrey Martin and Dr. John Mauro use premium intraocular lenses such as: Crystalens, Acrysof Toric, Restor and Technis.  Therefore we can not only correct far and near vision but astigmatism as well. What is a cataract? What to look for?

A cataract is a clouding the lens of the eye which, when were born is normally transparent. This clouding is usually caused by protein build up which makes up the lens begins to discolor and affect vision. The word “cataract” translates to “waterfall” and in early times it was said that people described there vision as if they were looking through a waterfall. When in the early stages, a cataract is usually mild and may only affect a portion of the lens. However,as the cataract grows larger it can make it tougher to  see.  Cataracts are like wrinkles in the skin, unfortunately as we get older, there inevitable and develop in both eyes.

There are several theories that exist that can cause cataracts including: smoking, diabetes, excessive exposure to sunlight, steroid use and certain other medications. The typical symptoms that may occur (blurry vision, glare around  lights at night, haloes, colors losing there contrast and increased nearsightedness. The increased nearsightedness is also cause a temporary improvement in close-up vision, but likely to worsen as the cataract continues to grow. Since cataracts tend to progress slowly, your vision changes are subtle and you may not notice any changes in your vision at first. When you see your eye doctor, a complete medical history and eye examination is necessary, which involves dilating your pupil to visualize the cataracts and the retina.

Individuals who are candidates for premium IOL implants at our Smithtown office practice usually present with  a cataract that is causing significant visual changes and are in otherwise in good ocular health. These patients also express  a strong desire to be less dependent on glasses for a wide range of vision, including distance and near. There are many different premium IOL manufacturers, which use different technology. After a full ophthalmic examination, any of our board certified doctors will discuss your vision goals with you. One of our doctors will then make a recommendation in terms of IOL choice that would be best fit their need. The most popular IOLs are the Crystalens®, ReSTOR®, ReZoom™, and most recently the Technis®.

The ReSTOR and the Technis are similar in that they are multifocal lenses. Meaning that at the center of the lens is a series of many stepwise concentric circles which are precisely calibrated to allow light to enter the eye, which leads to better focusing power in different distances. The ReZoom™ lens is made up of a few different zones, each to focus on a particular distance in certain lighted conditions. Finally, there is the Crystalens, which more simulates the human eye, designed to function in conjunction with the eye accommodating muscles. It has little hinges that allow it subtle movements that increase and decrease the power depending on the individual’s needs.

Cataract Surgery, What lens is best for me? Part one of two

The natural lens of the eye over time, becomes clouded. This is called a cataract. During the cataract procedure, the eye surgeon makes a small incision in the cornea about the size of a pencil tip. Then using ultrasound technology, the cataract is removed in a matter of minutes and carefully inserted is the new lens, either a monofocal or premium IOL(intraocular lens). A similar procedure is used to remove the natural lens for the patient who does not have cataracts but desires decreased dependence on glasses, either as an alternative to LASIK or PRK, they do not qualify for LASIK or PRK because there prescription is to large or to eliminate reading glasses in general. In this case, the patient may elect have a premium IOL. This is also called “clear lens exchange.” Drs. Zweibel, Martin and Mauro all use the "no-stitch" method of cataract surgery and implanting an IOL. The "no-stitch" technique allows fast visual recovery.

The intraocular lens serves as a replacement for the eye's natural lens.  Before the development of of IOL’s, patients had to wear “coke bottle” glasses after cataract surgery. First approved by the FDA in the 1980’s, the first line of IOL’s were called monofocal IOLs, which were and are still today used mainly to correct for distant vision. Premium IOLs bring about a new era in IOL technology, allowing good distance and near vision, without glasses for a majority of patients. Premium intraocular lens implants (IOLs) are an effective and safe way of restoring vision to patients undergoing cataract surgery.  Certain patients are candidates for a clear lens exchange. Even though they may not have a cataract, they may opt to replace the natural lens with a premium IOL to reduce their dependence on glasses. This by the way, also eliminates the need for future cataract surgery.

After surgery, with monofocal IOLs, patients still must use glasses in order to focus on  near activities such as reading and using a computer. Premium IOLs offer the ability for clear vision in  more than one distance without glasses. Tomorrow we will discuss different types of premium IOL’s and whether or not you may be a candidate.

Cataract surgery and blood thinners... ask the cataract experts at north shore eye care

North Shore Eye Care has been a leader in cataract surgery for many years.  The board certified eye doctors and eye surgeons at our smithtown new york office in long island are always trying to improve upon the state of the art outpatient surgery.  Cataract surgery is performed through a 2 millimeter incision using eye drops for anesthesia.  Our patients stay in their own clothes and return home the very same day.  We use IV sedation to keep our smithtown long island patients comfortable and our cataract surgeons perform cataract surgery in just a few minutes.  We specialize in premium implants that are designed to reduce or eliminate the need for glasses and contact lenses such as accommodating lenses like crystalens and the Acrysof Toric implant for the reduction or elimination of astigmatism.  We also use multifocal implants designed to make our suffolk county patients glasses independent.  The multifocal lens implants are the restor, rezoom, and tecnis multifocal. 

Some of our patients use blood thinners such as aspirin, coumadin, plavix or vitamin E for certain health problems.  These medications can be beneficial to blood flow and heart health, but they will make a patient bleed more freely.  In cataract surgery, there is usually no blood loss and therefore, stopping blood thinning medication is not a necessity.  However, our board certified eye doctors feel comfortable with stopping blood thinners if possible because there is always a small risk of bleeding.  That being said, Dr. Jeffrey martin, Dr. larry zweibel and dr. john mauro have performed cataract surgery on many patients on full anticoagulation without a problem.  It is important that you discuss blood thinners with your primary care doctor before stopping the use of these medications.
