One cataract setting that can be more difficult is cataract surgery in the setting of pseudoexfoliation.  The lens of the eye is held to the wall of the eye by small connective tissue strings called zonules.  Zonules create a netting that holds the lens steady as well as the implant lens or IOL following cataract surgery.  In pseudoexfoliation, the zonule apparatus is weak and the lens can be more loosely adherent.  Cataract surgeons depend on zonule support to complete cataract surgery safely.  In cases of pseudoexfoliation, our cataract doctors are ready with more gentle cataract removal techniques as well as supportive technology such as iris hooks and capsular tension rings.  With advanced technology and technique, most patients with pseudoexfoliation are able to have a routine safe cataract surgery.

In many cataract cases, our patients already have dry eye.  Dry eye can cause a gritty foreign body sensation or blurred vision with effort.  When watching a movie, reading a book, or driving a car, vision begins to blur after about 15 minutes.  Patients may also have a red irritated eye, excessive tearing, and itching.  Dry eye is a change that many women get due to menopausal hormonal changes.  Aging is a cause of dry eye.  Certain medications such as blood pressure medications, anti-depressants, and others cause dry eye.  Environmental changes can cause dry eye as well as irritants in the air.  With all this, cataract surgery will make dry eye worse for  a short time.  It is often necessary to treat dry eye postoperatively  especially when using premium implants.  These more complex implants require a good ocular surface for best performance.  The eye doctors at our Long Island New York practice are experts at treating dry eye.

Macular degeneration is a widespread eye condition that causes loss of central vision in patients.  Typically, macular degeneration is an eye problem of advancing age and is more typical in fair skin and light eyed patients.  Macular degeneration runs in families and tends to occur slowly; therefore regular eye examinations are important.  The eye doctors at North Shore Eye Care in Smithtown new york in Long island examine many patients with this eye problem and typically see drusen or calcification in the macula, which is the center of the retina, as the first sign.  Patient complain of metamorphopsia which describes distortion of a visual image.  There are treatments for macular degeneration including eye vitamins, laser and possible injections for wet macular degeneration.  Macular degeneration is dry when there is no bleeding under the retina and wet when new blood vessels form and start to bleed under the retina.  As patients age, cataracts develop as well.  A cataract is a clouding of the natural lens of the eye.  When a cataract develops in a patient with macular degeneration, it can be difficult to determine whether vision loss is attributed to the cataract or the damage to the retina.

Sometimes, the capsule that holds the new implant becomes cloudy over several months.  This is a normal part of healing.  Some patients heal and the capsule remains clear and others become only slightly hazy.  If the capsule clarity interferes with the quality of vision, we suggest Yag Capsulotomy.  With yag capsulotomy, we use a gentle laser right in the office to open up the central part of the capsule.  This is painless and takes about one minute.  About one third of patients that have cataract surgery require a yag capsulotomy.  If it is necessary, patients return to  normal activity immediately after the day of the laser.

Cataract surgery has changed dramatically over the last 20 years. There were no intraocular implants or IOLs so patients had to get very thick glasses called aphakic glasses so that they could see better.
