When you come to North Shore eye care with a complaint of dry eye that can include, burning, tearing, redness, itchiness, fluctuation in vision, vision worsening as the day progresses among other symptoms, we first get a tear lab.  Tear Lab is a computerized test that tell us the osmolarity of your tear.  Osmolarity stands for salt concentration and higher salt concentration means dry eyes.  We start with typical treatments like environmental modification and artifical tears.  If the osmolarity does not improve to normal range or symptoms abate, we consider further treatment including Restasis, which is topical cyclosporine that increases the health of the tear film and makes the eyes more comfortable.  We also consider small plastic pieces called punctal plugs. The purpose of the punctal plugs is to block the drainage of the tears from the ocular surface, keeping it better hydrated.  In our eye care practice, we have found that Restasis and punctal plugs work well together.  We also use steroid eye drops at times to further reduce inflammation that can be associated with dry eye.
